Complete and return to your relevant League Section Secretary, either Arvon Bickerstaff, Harold Gilmour, Derek McCann or Ken McCord by 30th April.

By email or post only, (details of both are included on the form).
Deadline for all entries: 30th April and cannot be accepted after this date.
Please note: Tom will not accept entries via a phone call.
